Purpose/Background: Colorectal cancer (CRC) incidence is reported to be declining overall in many countries but growing among young adults. The updated American Cancer Society (ACS) guidelines recommend screening starting at age 45 years. The aim of the study is to evaluate population-level trends in young colorectal cancer (yCRC) epidemiology in Qatar.

Methods/Interventions: Between January 2010 and December 2020, 1540 patients were included from the National Registry Database. The patients were divided into two groups. Group 1 (n = 384, ≤ 45 years) and group 2 (n = 1156, > 45). The epidemiological and clinicopathological features were compared between both groups.

Results/Outcomes: The overall male prevalence is 65.8%. The incidence of CRC in young patients was around 1 out of every 4 patients (24.6%). Interestingly, the poorly differentiated CRC (including mucinous and signet ring features) was more prevalent in group 1 compared to group 2 (19.5% vs. 7.3%) (p = 0.002). Advanced CRC stages (III, IV) were not significantly higher among yCRC, with 62.8% of the patients in group 1 diagnosed with advanced stages compared to 68.7% (p = 0.54). Young patients with CRC were found to have more rectal involvement, with 34.3% of the patients ≤ 45 having rectal cancer compared to 24.1% in patients > 45 (p = 0.001). Right side location was comparable between both groups (27.6% in ≤ 45 vs. 23.9% in > 45). (p = 0.15)

Conclusion/Discussion: Multiple studies are reporting an increase in prevalence of colorectal cancer among young patients. There is no standardized rate globally; however, the reported incidence rate is around quarter of the newly diagnosed patients in Qatar. Patients with yCRC have more aggressive and poorly differentiated histological type. Incidence of rectal cancer in young patients is higher than patients older than 45 years old.
