Purpose/Background: Albumin-myosteatosis gauge (AMG) is a novel marker that has recently gained interest in the field of oncology as a prognostic factor. A combination of serum albumin level and myosteatosis, AMG reflects both systemic inflammation as well as malnutrition status of a patient. Patients with sarcopenia and poor nutritional status are often at greater risk for complications after surgery due to impaired physical resilience and recovery capacity. Identifying sarcopenia-related factors has become increasingly important for assessing surgical risks in colorectal cancer patients. Although prognostic value of AMG have been investigated, no study has investigated its effect on postoperative complications. This study aims to explore the correlation between AMG and the development of postoperative complications in individuals undergoing colorectal cancer surgery.

Methods/Interventions: Patients who underwent open, laparoscopic, or robotic surgery for stage I-III colorectal cancer during March 2005 to February 2013 at a single tertiary hospital were reviewed. Data including demographics, perioperative parameters, preoperative albumin level, preoperative sarcopenia-related variables including skeletal muscle index (SMI), skeletal muscle density (SMD), skeletal muscle gauge (SMG), fat indices as well as albumin-myosteatosis gauge (AMG) were collected. Postoperative complications were categorized by Clavien-Dindo classification, and Grade III or higher were defined as major complication. Cut-off values for sarcopenia-related factors by sex were calculated. Univariable as well as multivariable analysis of factors associated with major complications were analyzed.

Results/Outcomes: Total number of 866 patients were analyzed. There were 348 (40.2%) females and 518 (59.8%) males, with 595 (59.8%) patients younger than 70 years old. Tumor location was subdivided into right colon (24.6%), left colon (44.6%), and rectum (30.8%). One hundred and eighty patients (20.8%) had postoperative complications of which 52 (6%) had major complication. Univariable analysis of factors associated with major complications were sex, age, tumor location, surgical method, operation time (min), SMI (cm2/m2), SMD (HU), SMG, visceral fat infex (VFI), subcutaneous fat index (SFI), albumin-bilirubin (ALBI) score, AMG, and albumin level. Multivariable analysis showed statistical significance in sex (p=0.006), tumor location (p < 0.001), surgical method (p=0.044), SMG (p < 0.001), VFI (p=0.004), SFI (p=0.040), and AMG (p=0.003).

Conclusion/Discussion: To our knowledge, this is the first study that investigated and demonstrated a significant correlation between AMG and the incidence of major postoperative complications in patients undergoing colorectal cancer surgery. Other known sarcopenia-related factors, such as SMG, VFI, SFI were also found to be independently associated with higher complication rates. These findings suggest that a comprehensive assessment of both inflammatory marker as well as nutritional status could be valuable for improving preoperative risk stratification and optimizing postoperative care in colorectal cancer patients. Further research is warranted to explore the potential of targeted interventions to reduce the risk of complications in this vulnerable population.
