Purpose/Background: Background: The incidence of early onset colorectal cancers (EOCRC), defined as colorectal cancer (CRC) ≤50 years old, has increased over the last 2 decades while incidence of CRC overall has decreased. Both the American Cancer Society (ACS) and United States Preventive Task Force (USPTF) have now recommended screening started at 45 years old, however the impact of earlier screening on addressing EOCRC is yet to be seen. The objective of this study was to identify racial disparities among EOCRC in our own county-hospital system.

Methods/Interventions:

Methods: We performed retrospective review of all patients >18 years old diagnosed with CRC during a 3 year time period. Patient were designated as EOCRC if their age was ≤50 years old at time of diagnosis. Chi square testing was used to identify associations with EOCRC.

Results/Outcomes: A total of 476 patients were included in the study, with average age of EOCRC of 42. Hispanic patients were overly represented among the EOCRC cohort (p=0.001). EOCRC patient were also more likely to be Stage III or Stage IV (p=0.05) and have poorly differentiated tumor grade (p=0.003) – Figure 1.

Conclusion/Discussion:

Conclusions: This preliminary study shows Hispanic patients are overly represented among EOCRC cases, consistent with data from previous studies. In addition, EOCRC patients are more likely to have diagnosis at later stage (III and IV) and are more likely to have poorly differentiated tumors. While longer term studies are needed to evaluate the impact of earlier screening, special attention should be used for thorough workup of younger patients, particularly Hispanic patients. Notably, given the average age of EOCRC patients is 42 years old, the current screening guidelines of 45 may still not capture a significant cohort of younger patients at risk of colorectal cancer.
