Purpose/Background: Robotic colectomies have grown significantly in recent years and are performed by both general surgeons (GS) and colorectal surgeons (CRS). The da Vinci 5 (DV5) platform stands out for its enhanced visual system, improved ergonomics, and force feedback (FFB) technology. This system senses tissue tension during procedures and provides objective postoperative performance metrics, offering surgeons valuable insights into their technique. Our study investigates whether differences in FFB occur during robotic colectomies based on the surgeon’s experience and training background.

Methods/Interventions: A retrospective analysis of low anterior resections (LAR) and right hemicolectomies (RH) using the DV5 platform was conducted between March and September 2024. Primary outcomes focused on the average force (in Newtons, N) applied across FFB settings (off, low, medium, high) by surgical specialty. Secondary outcomes included surgeon gender and experience.

Results/Outcomes: A total of 151 procedures (80 LAR, 71 RH) were analyzed. For LAR, CRS performed 73 cases (91.3%) and 7 cases (8.7%) were performed by GS. Compared with GS, CRS applied an average force of 2.162 N vs 1.559 N in the high setting, 2.648 N vs 3.355 N in the medium setting, 2.552 N vs 1.658 N in the low setting, and 4.876 N vs 4.253 N with FFB off, with an overall average force of 2.888 N vs 2.080 N (p = 0.2539). For RH, CRS performed 61 cases (86%), applying an average of 1.729 N vs 1.275 N in the high setting, 1.840 N vs 1.839 N in the medium setting, 1.853 N vs 1.511 N in the low setting, and 3.011 N vs 3.350 N with FFB off, with an overall average of 2.024 N vs 1.634 N (p = 0.026). Male surgeons applied higher average forces in LAR (2.981 N vs 1.981 N, p = 0.091) but lower average forces in RH (1.491 N vs 1.786 N, p = 0.201) compared to female surgeons. Experienced surgeons (>200 robotic cases) applied lower average forces in LAR (2.725 N vs 4.097 N, p= 0.0372) and RH (1.953 N vs 3.052 N, p= 0.3097) than less experienced surgeons.

Conclusion/Discussion: Preliminary data suggests a notable differences in FFB usage based on surgeon specialty, experience, and gender. CRS consistently applied higher forces than GS across most FFB settings during LAR and RH. More experienced surgeons used lower average force than novice surgeons, particularly in RH procedures. There was no statistical difference in forces based on gender. This study is one of the first that demonstrates that surgeon specialty, experience, and demographics may influence FFB usage, suggesting opportunities for optimizing training and improving surgical outcomes. Further research and data collection will need to be performed for further analysis.
