Purpose/Background: The incidence of colorectal cancer (CRC) among those aged < 50 years (i.e., early-onset colorectal cancer (EOCRC)) has increased by 1.4% annually since 2004 and continues to increase at an alarming rate. Given the reproductive potential of this group, CRC treatment poses unique challenges to reproductive health and family planning. In this qualitative study, we sought to explore these effects.

Methods/Interventions: We conducted semi-structured interviews with patients diagnosed with EOCRC (n = 41) across five diverse U.S. academic centers using convenience sampling. Data were analyzed iteratively, using steps informed by thematic content analysis.

Results/Outcomes: Among the 41 participants, the mean age was 43, with 54% female and 46% male. Ethnicities included 75% white, 10% Asian and 10% Black. Diagnoses varied, including colon cancer (39%), rectal cancer (54%), and colorectal cancer (7%). Three key themes emerged related to fertility and family planning for both male and female patients: (1) the impact of cancer treatment on fertility; (2) the prohibitive financial burden of fertility preservation; and (3) the emotional toll of fertility loss. Table 1 displays major themes with illustrative quotes.

Conclusion/Discussion: The impacts of EOCRC on fertility and health are multifaceted, encompassing factors such as the costs and risks associated with fertility preservation and the psychological effects on patients and their loved ones. Given the alarming rise in EOCRC, there is a critical need for affordable and accessible fertility preservation and family planning counseling.
