Purpose/Background: Life expectancy at 60s is 24,3 years in high income countries and health systems must face the burden of diseases of an elderly population and challenge the impact of treatments as major surgery. The aim of the study is to review the impact of colo-rectal surgery on elderly population comparing short-terms outcomes with younger population.

Methods/Interventions: All patients who underwent resective colorectal surgery (RCRS) at our institution between July 2022 and September 2024 were entered into a database. Preoperative, perioperative, and early (within 30 days) postoperative data were recorded. Patients were categorized into a young group (under 75 years, YG) and an elderly group (over 75 years, EG). A retrospective comparative analysis of postoperative outcomes was performed between the two groups; postoperative complications were graded according to Clavien’s classification.

Results/Outcomes: One-hundred fourteen and 219 patients were respectively in the EG and YG. Indications for RCRS was cancer in 79% of EG patients and 60.1% of YG patients and benign disease (complicated diverticular disease and inflammatory bowel disease) in 21.1% and 39.9% respectively (p < 0.001). The clinical presentation, localization, and rate of neoadjuvant treatment in oncological patients were comparable. About the baseline characteristics, the ASA and CCI scores were worse in the EG (p < 0.001). No significant differences about surgical approach, mini-invasive approach, conversion rate, definitive stoma creation, and number of harvested lymph nodes were found between the two groups. Overall, a post-operative complication within 30 days occurred in 108 patients (32.4%) but only 23 were grade ≥3 (6.9%). In EG the rate of post-operative complications was significantly higher than YG (41.2% vs 27.9%; p: 0.013), but no differences were found about rate of major complications (8.8% vs 5.9%; p: 0.334). In EG, a higher rate of patients (56.1% vs 22.8%; p< 0.001) needs of ICU in the immediate post-operative period. Three deaths were recorded (overall morbidity rate: 0.9%) all in EG; so, the short-term mortality rate was significantly higher in EG than in YG (2.6% vs 0%; p:0.016). The length of stay was significantly longer in EG than in YG (7.69+0.89 days vs 6.55+0.59 days; p:0.033).

Conclusion/Discussion: Advanced age does not seem to contraindicate RCRS, especially in colorectal cancer patients. The impact of elderly age on short-term outcomes seems to be minimal and acceptable.
