Purpose/Background: The patient is an 84-year-old male who presented with increasing abdominal girth for several weeks. He reported intermittent fatigue and dyspnea on exertion. Denied fever, night sweats, or recent weight loss. Labs without abnormal findings. CT imaging revealed circumferential thickening of the cecum with localized lymphadenopathy, as well as a lesion in segment five of the liver. Subsequent endoscopy showed a large necrotic ulcerated mass occupying most of the cecum, with biopsies indicating diffuse large B-cell lymphoma.

Patient has a history of diffuse large B-cell lymphoma of the right testicle, initially diagnosed September of 2008 and treated with Rituxan/CHOP and intrathecal methotrexate and radiation. This is his first recurrence.

Methods/Interventions: The patient case was discussed at multidisciplinary tumor board and recommended for surgical resection, due to risk of perforation. Patient underwent robotic-assisted laparoscopic ileocecectomy and biopsy of liver segment five.

Results/Outcomes: The patient underwent surgery and had an uneventful post-operative course. Final pathology from colon resection and liver confirmed diagnosis of diffuse large B-cell lymphoma. Patient was treated with systemic chemotherapy and 3-month post-operative PET CT-scan revealed no FDG-avid lesions. Patient remains in remission 15 months after surgery.

Conclusion/Discussion: Presentation of diffuse large B-cell lymphoma to the colon is a rare occurrence. Once diagnosed, the disease process typically responds well to systemic treatment. However, in this particular presentation of a bulky cecal mass, the risk of perforation during chemotherapy warranted a surgical approach.
