Purpose/Background: The treatment of Crohn’s perianal fistula (CPF) remains challenging due to its recurrent nature and resistance to complete healing. Polydeoxyribonucleotide (PDRN), a novel therapeutic agent with wound healing and anti-inflammatory properties, has been widely employed across various clinical fields. This study aims to assess the short-term outcomes of PDRN injection by comparing fistula closure rates and recurrence rates following CPF surgery with and without the administration of PDRN.

Methods/Interventions: A retrospective review was conducted on patients who underwent surgical procedures for CPF at a single center in Seoul, Korea, between January 2018 and June 2024. The study included cases of fistula closure operations, defined as surgical interventions intended to achieve closure of the fistula tract in a single step, such as fistulotomy, fistulectomy, and closure of the internal opening. Patients were categorized into two groups based on the application of local PDRN injection into the tissues surrounding the fistula tract during fistula closure operations. PDRN was uniformly injected around the fistula tract, as well as into the additional surgical wound following fistulotomy or fistulectomy. Clinical variables and postoperative outcomes, including fistula closure rates and recurrence rates, were compared between the two groups.

Results/Outcomes: Among the 47 fistula closure operations performed for CPF, 21 patients (44.7%) were assigned to the PDRN group, while 26 patients (55.3%) were allocated to the Without PDRN group. The distribution of operation types was 8 (38.1%) vs. 9 (34.6%) in the fistulotomy and fistulectomy group, and 13 (61.9%) vs. 17 (65.4%) in the closure of internal opening group. The mean follow-up duration was 7.67±8.06 months, with no significant differences in baseline characteristics, including age and sex, between the two groups. Notably, patients in the PDRN group demonstrated a significantly higher rate of complete fistula closure (p=0.007; 88.5% vs. 64.7%), a shorter time from surgery to complete fistula closure (p=0.044; 3.38±2.60 months vs. 5.92±3.86 months), and a shorter follow-up interval (p=0.002; 4.67±3.74 vs. 11.54±4.67) compared to the Without PDRN group. No significant difference was observed in the rate of fistula recurrence between the two groups. In multivariate analysis, PDRN was identified as the only significant factor associated with the increased rate of complete fistula closure (HR 2.615, 95% CI 1.218-5.614, p=0.014).

Conclusion/Discussion: PDRN is a novel therapeutic agent that presents a viable option for CPF surgery, improving the rate of complete fistula closure through a cost-effective, safe, and relatively simple procedure. To the best of our knowledge, this study is the first to underscore the potential of PDRN in the treatment of CPF.
