Purpose/Background: Desmoid-type fibromatosis (DF) is a rare subtype of soft tissue neoplasms known for its locally aggressive behavior and fibrotic consistency. While these tumors can arise in various anatomical sites, their occurrence within the colon is exceptionally rare and poses diagnostic and therapeutic challenges.

Methods/Interventions: We present a case of a 36-year-old African-American female with no past medical history who presented with vague gastrointestinal and urinary symptoms. Imaging studies revealed a sizable abdominal mass, initially raising uncertainty regarding its origin. Subsequent exploratory laparotomy revealed a desmoid fibromatosis involving the sub-serosa and muscularis propria of the cecum, prompting a right hemicolectomy.

Results/Outcomes: Histopathological analysis confirmed the diagnosis, and the patient underwent a successful surgical resection with negative margins. Given the high risk of recurrence associated with DF, a multidisciplinary approach was adopted involving close surveillance, and consideration for adjuvant therapies such as radiation.

Conclusion/Discussion: In light of the rarity of colonic desmoid tumors, this case report offers a valuable framework for their management. It highlights the necessity of individualized treatment approaches and ongoing surveillance, which are essential for achieving optimal outcomes and promoting disease-free survival.
