Purpose/Background: The management of perforated diverticulitis has historically centered on Hartman’s procedure or resection with primary anastomosis and diverting loop ileostomy. These operations have demonstrated similar morbidity and mortality, however patient’s undergoing Hartman’s procedure have a lower ostomy reversal rate. With this knowledge, what factors contribute to a surgeon’s choice of operation? We aimed to determine if the time of day at which the operation began, or the American Society of Anesthesiologist (ASA) classification, affected the operation a patient received for perforated diverticulitis of the descending colon.

Methods/Interventions: Using our National Surgical Quality Improvement Program (NSQIP) database, we conducted a retrospective review of 79 patients undergoing surgery for perforated diverticulitis of the descending colon at Berkshire Medical Center from January 1, 2018 to January 1, 2024. Operations beginning during the hours of 0600-1759 were identified as, “daytime operations”, and cases starting between 1801-0559 hours were identified as, “nighttime operations”. Furthermore, the operations were defined as either Hartman’s procedure or resection with primary anastomosis and diverting with loop ileostomy.

Results/Outcomes: A total of 50 Hartman’s procedures were performed (63.3%), and 29 operations with diverting ileostomy’s were performed (36.7%). A total of 47 daytime cases (59.5%) were performed, with 32 nighttime cases (40.5%) performed. The time of the surgery did not affect the procedure performed (chi-square = 1.71, with 1 degree of freedom, p = 0.19). Mean ASA classification between groups was similar (Hartman’s = 2.84 ± 0.68 vs anastomosis and proximal diversion = 2.72 ± 0.53; p = 0.43) and did not affect the choice of procedure.

Conclusion/Discussion: In a small, community-based rural teaching hospital, without a dedicated acute care surgery service, the time of day at which an operation began for perforated diverticulitis of the descending colon did not affect the choice of surgical procedure performed. Furthermore, ASA classification did not affect the choice of procedure either.
