Purpose/Background: Children presenting with inflammatory bowel disease (IBD) often present with severe disease, both based on age of diagnosis and endoscopic findings. We aimed to investigate the rates of operative intervention for children with IBD based on age of diagnosis.

Methods/Interventions: After institutional review board approval, we queried the ImproveNowCare database for our institution to obtain all patients diagnosed with IBD before 18 years of age, who have been seen at our center from 2016-2024. Using electronic health record data, we then identified the age of diagnosis, pathology, requirement and type of operative intervention, and age of initial an operation. Results were analyzed to identify operative incidence and lead time from diagnosis to operative intervention.

Results/Outcomes: Overall, 224 pediatric patients with IBD were identified: In this group, 67% (n=150) carried a diagnosis of Crohn’s Disease, 29% (n=65) of ulcerative colitis, and 4% (n=9) of indeterminant colitis. Of the 9 patients diagnosed with IBD before the age of 5, 1 required an operation, 13 years after diagnosis. Those diagnosed between 5 and 10 years of age had an 8.7% operative incidence, while 16.2% of children diagnosed between 10 and 15 years old required operative care. Additionally, 25% of children diagnosed from 15 to 18 years old required an operation on average within 1 year of diagnosis. (Figure).

Conclusion/Discussion: Patients diagnosed with IBD in their teenage years are more likely to require an operation than their younger counterparts, and an operation is more likely to occur within 1-2 years of their diagnosis. The reasons for their acute surgical presentation should be further investigated and a colorectal surgeon should be included in their multidisciplinary care team, given the high rate of operative intervention required in this age group.
