Purpose/Background: Colorectal cancer represents the third most common cause of cancer among men and women in U.S.- Although survival has significantly increased due to improved diagnosis and treatment of primary tumor, proper surveillance is still warranted to evaluate for any degree of recurrence. Several established barriers to proper follow up have been documented in the literature including no insurance, higher stage tumor, older age, and living without a partner. Our study seeks to evaluate demographic and outcome differences amongst stage II CRC patients who did or did not receive proper follow up.

Methods/Interventions: IRB approved retrospective evaluation of patients from 2011-2020 who underwent resection for stage II colon cancer at single center within one network in central Ohio. Patients were then categorized as either having appropriate or inappropriate follow up based on established follow up guidelines (clinical examinations at 3 and 6 months, CEA levels at 3 and 6 months, colonoscopy 1 year after resection, annual CT chest abdomen pelvis)

Results/Outcomes: 138 total patients evaluated, 52 of which were categorized as having appropriate follow up. No significant differences in sex, BMI, ASA class, or race noted between groups. Patients with inappropriate follow up were less likely to be employed and to have private insurance (p < 0.05). Patients with inappropriate follow up were significantly less likely to have high risk cancers, and found to have lower overall recurrence. Cancer related death was greater in the inappropriate follow up group and approached significance (p=0.06)

Conclusion/Discussion: Patients with significant risk factors such as lack of full time employment and private insurance given that these groups were significantly more likely to have sub standard follow up. Recurrence was significantly lower in the poor follow up group likely related to the decreased frequency of evaluation. Additionally, patients with substandard follow up were less likely to have high risk features; potentially why these patients were not followed as meticulously. Cancer related death was higher in patients with poor follow up and approached, but did not reach significance. This may be related to overall power of the study and is something to be investigated further.
