Purpose/Background: Colorectal cancer (CRC) is a leading cause of morbidity and mortality worldwide, including in Mexico, where it ranks as the third most common cancer in both men and women. Its incidence has been rising in recent decades due to factors such as poor dietary habits, sedentary lifestyles, tobacco use, and genetic and environmental influences. Despite advances in screening and treatment, many cases are diagnosed at advanced stages, which complicates therapeutic interventions and reduces survival rates. Early detection and lifestyle modifications, such as improved diet and increased physical activity, are essential to combat the growing burden of CRC. Public health efforts must focus on raising awareness, promoting screening, and addressing modifiable risk factors to improve outcomes for patients.

Methods/Interventions: A retrospective observational study was conducted, including 2,263 patients with a confirmed diagnosis of CRC between 1988 and 2023. Data were obtained from the hospital's medical records. Collected data include gender, age, tumor location, intestinal phenotype, tumor differentiation grade, and cancer stage. To evaluate the evolution and characteristics of CRC, patients were divided into two periods: 1988-2005 and 2006-2023, and into age groups: under 35 years and over 35 years. Data analysis was performed using the SPSS statistical package version 26.

Results/Outcomes: This study analyzed 2,263 patients, with 47.6% women and 52.4% men. Of these, 23.81% were under 45 years old, and 76.19% were over 45, with a mean age of 56 years. The most frequent tumor location was the rectum (51.1%), followed by the right (27.7%) and left colon (21.2%). The predominant cancer type was adenocarcinoma (92.1%), with 54.8% of tumors moderately differentiated. Most patients (77.3%) were diagnosed at advanced stages (III-IV).
A sub-analysis of two periods (1988-2005 and 2006-2023) revealed no significant changes in patient demographics. However, tumor location distribution shifted, with the right colon showing a decline (from 40% to 24.4%) and the left colon increasing (from 9% to 24.5%). Tumor differentiation significantly improved, with a higher percentage of well-differentiated tumors in 2006-2023 (32.7%) compared to 1988-2005 (26.4%). Disease stages remained largely consistent, with most patients diagnosed at advanced stages in both periods.
In a further analysis of patients under and over 35 years, younger patients saw an increase in advanced-stage diagnoses over time. Additionally, the proportion of younger patients decreased significantly from 10.3% in 2005-2014 to 7.5% in 2015-2023.

Conclusion/Discussion: This study presents a comprehensive analysis of colorectal cancer (CRC) trends over 35 years in a Mexican 3rd level hospital. It highlights a slight male predominance (52.4%) and an average patient age of 56 years. Although CRC mainly affects those over 45, 23.81% of cases occurred in younger individuals, emphasizing the need for early detection in this group. The most common tumor location was the rectum (51.1%), and adenocarcinoma was the predominant phenotype (92.1%). Most patients (77.3%) were diagnosed at advanced stages (III-IV), reflecting a trend seen globally. The study calls for the expansion of screening programs and improved early detection, especially in younger patients, as CRC rates in those under 35 have increased in recent years. Improved access to colonoscopies and further research on genetic factors are essential to reducing CRC incidence and mortality.
