Purpose/Background: The incidence of young onset colorectal cancer has increased. However, the associated clinical outcomes was not clear. The aim of our study was to evaluate their clinical features and prognosis.

Methods/Interventions: This retrospective cohort study was based on the multi-institutional Chang Gung Research Database. The patients with colorectal cancers, who received treatment in Chang Gung Memorial Hospitals were included from 2007 to 2017. The definition of young onset colorectal cancer is that the patients were under the age of 50 years.

Results/Outcomes: A total of 16990 colorectal cancer patients were included and 2312 (13.6%) were under the age of 50 years. The clinical features, which were predominant in the patients ≥ 50 years of age, included male gender, body mass index, CEA, low hemoglobin, hypoalbuminemia, and right sidedness. The patients under the age of 50 years had more high neutrophil-lymphocyte ration (NLR), advanced stage and chemotherapy rate. From stage 0 to stage III, the patient < 50 years of age were associated with better disease free survival and overall survival. In stage IV, the young onset colorectal patients had poor disease free survival and overall survival, which contributed the poor survival in all stages patients analysis.

Conclusion/Discussion: Young onset colorectal cancer patients had better clinical features and high chemotherapy rate. However they had more stage IV, which were associated with poor survival.
