Purpose/Background: Inflammatory bowel disease (IBD) significantly affects sexual function due to symptoms such as diarrhea, fatigue, and abdominal pain. These negative effects on sexuality and intimacy are among the main concerns of patients. However, there is limited data on the prevalence and characteristics of sexual dysfunction and erectile dysfunction in this population, highlighting the need for further research in this area.

Methods/Interventions: A cross-sectional study was conducted, including 45 consecutive patients from the Inflammatory Bowel Disease Clinic at the General Hospital of Mexico “Dr. Eduardo Liceaga.” It included 19 female patients, 14 diagnosed with idiopathic chronic ulcerative colitis (CUCI) and 5 with Crohn's disease (CD) and 26 male patients, 21 diagnosed with CUCI and 5 with CD. The Female Sexual Function Index (FSFI) was used to assess sexual function in women, examining six domains: desire, arousal, lubrication, orgasm, satisfaction, and pain. For men, the International Index of Erectile Function (IIEF) was used, which evaluates five domains: erectile function, sexual satisfaction, sexual desire, overall satisfaction, and orgasmic function. Clinical and demographic data were collected for each patient to provide a comprehensive characterization of the sample.Statistical analysis was conducted using SPSS version 29.

Results/Outcomes: In the study on sexual function in IBD patients, it was found that 63.2% of women did not experience sexual dysfunction, while 36.8% experienced some degree of dysfunction. Among men, 73.1% had some form of erectile dysfunction, significantly higher than the 26.9% who had no dysfunction. Of the women, 73.7% had CUCI. Most were in the inactive phase (58.33%) and had Montreal E3 classification extension (47.4%). A total of 26.3% had CD, mainly classified as Montreal A3, B2, L3. Among men, 80.8% had CUCI, with 57.6% in the inactive phase, and most had Montreal E3 extension (47.36%). The remaining 19.2% had CD, showing greater severity of sexual dysfunction, particularly in patients classified as Montreal A2, B2, L4. Table 1 presents the demographic and clinical characteristics and sexual function evaluation of IBD patients included in the study.

Conclusion/Discussion: Our study shows that while the majority of women with IBD did not experience sexual dysfunction, a high proportion of men, particularly those with CD, experienced erectile dysfunction. These findings emphasize the importance of evaluating and addressing sexual dysfunction in IBD patients to improve their quality of life. Additionally, the variability in the severity and type of sexual dysfunction among different IBD subgroups highlights the need for personalized approaches in the clinical management of these patients.

Demographic and Clinical Characteristics of Patients with Inflammatory Bowel Disease (IBD) Included in the Study
Table 1 presents the demographic and clinical characteristics of 45 patients with Inflammatory Bowel Disease (IBD), including the evaluation of sexual function using the FSFI for female patients and the IIEF for male patients. The outcomes of sexual dysfunction are categorized by severity in both genders, and further stratified by diagnosis (Ulcerative Colitis or Crohn’s Disease), Montreal classification, course of the disease, and type of treatment.
