Adenocarcinoma is the most prevalent type of cancer affecting the colon and rectum. In contrast, carcinoids are rare, slow-growing asymptomatic neuroendocrine tumors often discovered incidentally during a rectal exam or endoscopy. The occurrence of a rectal carcinoid in colon adenocarcinoma is rare with incidence in case reports. There have been no known reports from the Philippines especially in synchronous cases with early neuroendocrine tumor presenting with an internal lymph node metastasis. For most patients identified, the prognosis is primarily influenced by the adenocarcinoma rather than the carcinoid tumor however lymph node metastasis in rectal carcinoid tumor may affect prognosis of the carcinoid as well hence affects management and surveillance. This article discusses a case of sigmoid adenocarcinoma with an incidental rectal carcinoid with lateral lymph node metastasis
