Purpose/Background: To assess the Efficiency and safety of an Energy device, acombined Ultrasonic and bipolar energy device, in controlling and sealing vascular pedicles in colorectal resection.

Methods/Interventions: All patients who underwent colorectal cancer resection between January 2021 and December 2023 were included in this study. Non-standard colorectal resections were excluded. Patient demographics and operative details, including intraoperative bleeding, haemoglobin drop, and postoperative complications, were recorded. Moreover, a subgroup was created for laparoscopic-only cases, and the energy device was compared to Haemolock (surgical clips)clips. Analysis was performed with SPSS 27 software.

Results/Outcomes: A total of 288 patients were included in this study, and the energy device was the method of haemostasis control and vessel sealing in 210 (72.9%) patients. Of these 210 patients, 41.9% underwent right hemicolectomy, and 25.7% underwent anterior resection. The vessel pedicles sealed were ileocolic (42.1%), inferior mesenteric (53.1%), middle colic (3.4%) and more than one pedicle (1.5%). The mean of intraoperative blood loss was 163.3 ml. Regarding postoperative complications, anastomosis leak was observed in 1.6%, abdominal collection was in (4.3%), surgical site infection (5.8%) and no patients underwent reoperation for bleeding. Subgroup analysis for laparoscopic-only patients (n=200) showed significantly higher intraoperative bleeding in the patients that Haemolock was used to control the pedicle when compared to the energy device group (Thunderbeat) (P=0.009); however, no difference was observed regarding haemoglobin drop (P=0.80).

Conclusion/Discussion: The combined Ultrasonic and bipolar energy device is safe in most vascular pedicles in colorectal cancer resection and reduces intraoperative blood loss. Randomised controlled trials are needed to provide robust evidence for energy devices in colorectal surgery.
