Purpose/Background: The aim of this study was to demonstrate the impact of superior rectal artery (SRA) preservation on anastomosis leakage and recurrence-free survival in laparoscopic anterior resection for sigmoid colon cancer.

Methods/Interventions: Data from patients with sigmoid colon cancer who underwent laparoscopic anterior resection between January 2015 and December 2019 at Severance hospital, Seoul, Korea were retrospectively reviewed. Patients who underwent anterior resection with SRA preserving technique were classified as proximal anterior resection (PAR) and patients who underwent anterior resection with conventional high or low ligation of inferior mesenteric artery technique were classified as distal anterior resection (DAR).

Results/Outcomes: Among 66 patients, 11 (16.7%) were in the PAR group and 55 (83.3%) were in the DAR group. Age, sex, body mass index (BMI), and American society of anesthesiologists score were not different between groups. There was only one patient who experienced postoperative anastomosis leakage in the DAR group. However, there was no difference between the two groups in the overall complication rates. Four patients (7.3%) in the DAR group and one patient (9.1%) in the PAR group experienced recurrence. There was no significant difference in the mean recurrence-free survival between the two groups (53.7 vs. 57.9 months, p=0.455).

Conclusion/Discussion: SRA preserving technique seems to be a feasible treatment option in patients undergoing laparoscopic anterior resection for proximal sigmoid colon cancer.
