Purpose/Background: Many studies have reported that complete mesocolic excision (CME) provides an oncologic benefit for patients with colon cancer. Most recently, the randomized trial RELARC found a higher rate of 3-year disease-free survival (DFS) in patients with node-positive colon cancer who underwent a laparoscopic CME with D3 lymphadenectomy compared with patients who underwent a D2 lymphadenectomy (76% vs 66%). We set out to analyze outcomes in patients who underwent a robotic CME with a D3 lymphadenectomy at our center over a 10-year period.

Methods/Interventions: A prospectively maintained institutional database was searched to identify patients who underwent a robotic right hemicolectomy with CME and D3 lymphadenectomy performed by either of two experienced, high-volume surgeons in 2014–2024. Data on DFS, complications, and readmissions were collected from electronic medical records.

Results/Outcomes: A total of 631 patients (median age 66 years, IQR 55-74) underwent a robotic right hemicolectomy with CME and D3 lymphadenectomy for nonmetastatic colon cancer. Median operative time was 165 min (IQR 140–165); median blood loss was 25 ml (IQR 25–25). No patient experienced an intraoperative vascular injury or underwent conversion to open surgery. Postoperative complications of Clavien-Dindo grade 3 or higher occurred in 15 patients (2.4%) (Table), including anastomotic leaks in 7 patients (1.1%), with 4 (0.6%) of them requiring reoperation. Three patients (0.4%) experienced thrombosis in the superior mesenteric vein following discharge and received anticoagulation treatment. Median length of stay was 4 days (IQR 3–4). Thirty-eight patients (6.0%) were readmitted within 30 days. Two patients (0.3%) died within 30 days of discharge. The median number of lymph nodes harvested was 30 (IQR 22–30). Five-year DFS was 98.6%, 90.5%, and 70.2% for patients with AJCC stage, I, II, and III, respectively. With median follow-up of 39.6 months, local (anastomotic) recurrence developed in 2 patients and distant recurrence developed in 68 patients.

Conclusion/Discussion: Robotic right hemicolectomy with CME and D3 lymphadenectomy has a low rate of complications and may be associated with longer stage-specific DFS in comparison with open or laparoscopic D3 lymphadenectomy.
