Purpose/Background: The purpose of this study was to analyze the clinical outcome of patients treated with primary fistulotomy and drainage of pus in perianal abscess after undergoing endoanal ultrasound/MRI pelvis

Methods/Interventions: The study was done from August to 2024 July in 448 patients diagnosed to have perianal abscess,endoanal ultrasound/MRI pelvis done for all the patient and they were classified into ischiorectal abscess, perineal abscess, submucosal, supralevator and intersphincteric abscess along with abscess cavity, fistula tract with internal opening was noted for most of the patients. fistulotomy was done for all the patient pus drained and was given for the abscess cavity.

Results/Outcomes: Data analysis was based on reviewing hospital record and questionnaires.evaluation was done for recurrence of abscess, fistula formation,incontinence and postoperative recover period
Based on this study identifying internal opening of fistula tract in perianal abscess using endoanal ultrasound it was 100 percentage and for MRI it was 98 percentage,recurrence of abscess was seen in 3 patient,fistula/sinus formation was seen in 5patient,mean recover period was 4days and incontinence was seen in 3patients.

Conclusion/Discussion: Endoanal ultrasound/MRI pelvis must be done for all patient with perianal abscess to identify the internal opening and fistula tract. Primary fistulotomy should be done in all patients along with abscess drainage to avoid formation of fistula in ano.
