Purpose/Background: The aim of this study was to compare the outcomes of laparoscopic surgery with complete mesocolic excision (CME) versus open surgery for right colon cancer.

Methods/Interventions: PubMed, Science Direct, Cochrane, Proquest, JSTOR, and Google Scholar were searched. Primary outcome was survival rates. Secondary outcomes were local or systemic recurrence, intraoperative morbidity, postoperative complication, and hospital stays

Results/Outcomes: Twenty studies were identified. The 5-year survival rate was higher for laparoscopic surgery compared to open surgery (up to 94.7% vs. 83.7%) and for CME procedures compared to non-CME (up to 95.2% vs. 86.1%). Patients under 80 years old had higher survival rates compared to those over 80 (79.3% vs. 67.7%). The 3-year survival rates, reported in six studies, generally favored laparoscopic over open surgery and CME over non-CME, with variations. Recurrence rates were lower for CME compared to non-CME (10.2% vs. 18%). Local recurrence rates were lower for laparoscopic CME compared to open CME (0.9% vs. 3.1%). Laparoscopic CME was associated with longer operative times, but reduced blood loss and shorter hospital stays compared to open procedures. Postoperative complications at 30 days were generally lower for laparoscopic CME

Conclusion/Discussion: Laparoscopic CME shows superior long-term survival rates and lower recurrence rates compared to open surgery and non-CME procedures. While laparoscopic CME procedures are longer, they result in less blood loss and shorter hospital stays. These findings support the use of laparoscopic CME for improved outcomes in right colon cancer surgeries.
