EP908 - Clinical, biochemical and surgical indicators associated with postoperative complications in patients with colon trauma

Purpose/Background: This study is conducted with the aim of analyzing the criteria associated with an increase in postoperative complications in patients with colon trauma in an emergency center, since colon trauma is a relatively frequent entity in our environment, with a target population in young labor-active people, and is associated with complications in most cases, whether complications associated with general condition, closure of the lesions, septic state of the abdominal cavity or associated with hospitalization.

The aim of this study is to analyze the sociodemographic and anthropometric data of the society with postoperative complications associated with colon trauma, which are the most frequent complications associated with colon trauma, as well as the clinical, biochemical and surgical criteria that increase the morbidity associated with this condition.

Methods/Interventions:: This is a retrospective and analytical analysis of patients admitted with a diagnosis of penetrating or blunt colon trauma to this unit in a period of two years, exclusion and inclusion criteria were applied, obtaining a total of 47 patients.

Results/Outcomes: In conclusion, there are multiple risk factors that individually or in association with others increase morbimortality in patients with abdominal trauma, specifically colonic trauma. Among the most important risk factors are hospital stay greater than 10 days, ISS score greater than 25 points and management with colostomy.
There is no relationship in this population sample between morbidity and factors such as sex, presence of chronic degenerative, surgical or drug addiction history, type of trauma, Glasgow scale on admission, injured colon region, local peritonitis, presence of hemoperitoneum, hemoglobin, neutrophils, total bilirubin, creatinine, electrolytes, CRP on admission, pH on admission, body temperature on admission, or the performance of abdominal lavage during the trans-operative period.

Conclusion/Discussion: According to the findings obtained in this study, the following is recommended: Avoid the use of stomas whenever there is no absolute indication, optimize transfer to reduce the delay from the time of trauma to hospitalization in patients with abdominal trauma, appropriate management of septic shock from the first contact with the emergency department, early initiation of the oral route, i.e., during the first three days if there are no contraindications, avoid prolonged hospital stay by improving the multidisciplinary processes necessary for adequate management and satisfactory evolution, calculation of APACHE II, ISS and Flint scale, to predict the risk of complications and initiate adequate management for their secondary prevention.
