Purpose/Background: Over the past few decades, advances in minimally invasive surgery have revolutionized the field of colorectal surgery, with both laparoscopic and robotic approaches gaining prominence. The purpose of this study was to compare outcomes and costs of robotic versus laparoscopic colectomy throughout the US.

Methods/Interventions: The Nationwide Readmissions Database for 2016-2020 was queried for all patients undergoing minimally-invasive colectomy. Patients undergoing robotic colectomy were compared to patients undergoing laparoscopic colectomy. Propensity matching was performed one to one using predictors for age, gender, colonic malignancy, comorbidities, elective surgery, hospital characteristics, and socioeconomic factors. The primary outcome was mortality. The secondary outcomes were venous thromboembolism (VTE), surgical site infection (SSI), length of stay (LOS), and cost of hospitalization.

Results/Outcomes: There were 140,986 patients undergoing minimally invasive colectomy and successfully propensity matched. There were 581 (0.4%) patients who died and there was no statistically significant difference in mortality between robotic and laparoscopic surgery. There were 1,034 (0.7%) patients with a VTE and there was no difference between robotic and laparoscopic surgery. There were 150 (0.1%) patients with an SSI and the risk was increased with robotic surgery (OR 1.5 [1.1-2.2] p=0.01). The mean LOS was shorter for robotic surgery (4.5 ±4.9 versus 5.0 ±5.2 days, p< 0.001) and mean hospitalization cost was higher for robotic surgery ($24,613 ±$18,165 versus $20,382 ±$17,533, p< 0.001).

Conclusion/Discussion: While robotic surgery demonstrated a shorter length of stay, it did not offer significant improvements in mortality or major complications such as venous thromboembolism. The increased risk of surgical site infection and higher costs associated with robotic surgery highlight the need for careful patient selection and consideration of economic factors in surgical decision-making.
