Purpose/Background: The adoption of robotic techniques in emergency operations has been slowly expanding. The data to support these techniques in emergency colorectal procedures for diverticultiis is however lacking. Thus, we conducted a study to evaluate the value of robotics in emergency surgery for diverticulitis.

Methods/Interventions: We analyzed the American College of Surgeons National Surgical Quality Improvement database for the targeted procedure Colectomy in the year of 2022. We only included cases of acute diverticulitis treated emergently with evidence of perforation. We conducted Poisson Regression and linear regression models adjusting for age, preoperative albumin level, weight, sex, ASA score, smoker status, and diabetes. Our primary outcomes were the incidence of unplanned conversions and adverse events - including renal failure, blood transfusion, respiratory and cardiac complications, postoperative sepsis, deep and superficial surgical site infection. Secondary outcomes include unplanned readmission, length of in-hospital stay and anastomotic leak.

Results/Outcomes: We identfied 753 cases of emergent robotic colectomies for perforated acute diverticulitis. Our other MIS group included 3,831 patients. The two groups were well matched for age, gender, ASA, and comorbidities. In our analysis we noteda significant reduction in the incidence of unplanned conversions with robotic approach after multivariate analysis (adjusted RR 0.56, 95 % CI {0.42 - 0.80}, p=0.001. Similarly, there was a significant reduction in postoperative morbidity (adjusted RR 0.71, 95 % CI {0.58 - 0.88}, p = 0.001) when the procedure was performed robotically.
We observed a significant decrease in anastomotic leak (adjusted RR 0.76, 95 % CI {0.67 - 0.86}, p< 0.001 and in total length of in-hospital stay (mean 4.23 vs 5.34 days, p< 0.001) for the robotic approach. However there was no significant difference in unplanned readmission (adjusted RR 0.77, 95 % CI {0.57 - 1.03}, p = 0.086).

Conclusion/Discussion: Robotic approach in the emergy setting for perforated acute diverticulitis is associated with decreased conversion rate, fewer adverse events, and shorter length of stay. These preliminary data support additional efforts to expand the role of robotics in emergency colorectal surgery.
