Purpose/Background: Diverticulitis affects almost 200,000 patients each year and the minority of these patients will require surgical intervention. Multiple risk factors have been described for patients who might require surgery. Multiple clinical trials have shown the efficacy of supportive care. This retrospective study aims to examine the rate of surgery in patients after a diagnosis of colonic diverticulitis using the global TriNetX patient database.

Methods/Interventions: The TriNetX system uses de-identified patient data from the Global Collaborative Network, a database of over 110 million patients. We constructed cohorts based on the interventions they received within a 6-month period after being diagnosed with their first episode of diverticulitis. Outcome for each cohort was if the patient underwent colon resection at any point within defined time points. The percentage of each cohort that underwent colon resection 1, 2, 3, and 5 years after a 6-month treatment period was measured. Cohort A: patients received supportive care only and did not receive antibiotics, IR drainage, or surgery at initial presentation. Cohort B: patients who received only antibiotics but no IR drainage or surgery. Cohort C: patients received antibiotics and IR drainage but no surgery.

Results/Outcomes: The cohorts showed no significant differences noted in regards mean age at time of diagnosis, gender, or race. The risk of surgery within each cohort at 1, 2, 3 and 5 years, respectively, are as follows: 2.1%, 2.9%, 3.3%, and 3.9% in Cohort A (272,805 patients); 4.4%, 5.4%, 6.0%, and 6.6% in Cohort B (244,746 patients); 13.0%, 15.0%, 15.7% and 16.3% in Cohort C (4,892 patients). The risk differences between Cohorts A and B at all time points, and those between Cohorts A and C at all time points were statistically significant. P-values for all comparisons were less than 0.0001.

Conclusion/Discussion: With the ability to query a global patient database, we report rates of colon resection in patients diagnosed with diverticulitis of large sample size at 5 years, stratified by the treatment they received during their index episode. Patients who were successfully managed without antibiotics had a relatively low risk of requiring surgery at 5 years. This risk was significantly increased in patients who required IR drainage and can be used to counsel patients after presenting with their first episode of diverticulitis.
