Purpose/Background: Colorectal cancer liver metastases (CRLM) are one of the most frequent pattern of recurrence and metastasis, yet the impact of background liver fibrosis in influencing recurrence rates remains unclear. This study aimed to histopathologically assess and quantify fibrotic changes in resected CRLM specimens to evaluate their impact on the risk of intrahepatic recurrence.

Methods/Interventions: A total of 102 patients who underwent liver resection for CRLM at our institution between 1999 and 2022 were included. Fixed specimens of liver resection samples were evaluated with EVG staining, and the extent of fibrosis in the background liver tissue was quantified via whole-slide imaging (WSI). Based on the fibrosis levels, patients were categorized into two groups: with fibrosis (n=24) and without fibrosis (n=78), using Classification and Regression Tree (CART) analysis. Further multivariate analysis was performed using the Cox proportional hazards model, with fibrosis treated as a continuous variable, to assess its association with residual liver recurrence.

Results/Outcomes: Residual liver recurrence occurred in 36.3% of patients overall, with a significantly higher rate in the fibrosis group (58.3% vs. 29.5%, p=0.019). No significant difference in extrahepatic recurrence, including peritoneal dissemination and lung metastasis, was observed between the two groups (41.7% vs. 43.6%, p=1.0). Multivariate analysis revealed that the quantitative fibrosis score was an independent risk factor for residual liver recurrence (OR 1.36, 95% CI 1.01–1.85, p=0.043), indicating a linear relationship between fibrosis severity and recurrence risk.

Conclusion/Discussion: Background liver fibrosis was identified as an independent prognostic factor that increased the likelihood of residual liver recurrence following CRLM resection. These findings suggest that patients with significant fibrosis may benefit from more meticulous surgical strategies. Further investigation is warranted to clarify the role of fibrosis in tumor cell growth and proliferation.
