Purpose/Background: Low anterior resection syndrome (LARS) includes various bowel dysfunctions after rectal cancer surgery, significantly impairing quality of life. Although symptoms may improve over time, it remains unclear which symptoms show the most improvement. This study aims to evaluate symptomatic differences in LARS based on postoperative time intervals.

Methods/Interventions: A cross-sectional study was conducted on patients who underwent sphincter-saving surgery for rectal cancer between 2009 and 2016. Patients were divided into three groups: < 2 years, 2-5 years, and >5 years post-surgery. The LARS score and Cleveland Clinic Incontinence Score (CCIS) were compared across these groups, and univariate analysis of individual questionnaire items was performed to identify symptom variations.

Results/Outcomes: A total of 216 patients were included. Patients >5 years post-surgery reported mild frequency of 1-3 times per day significantly more than the others. Clustering and incontinence for flatus and liquid stool were similar across all groups. The CCIS total score improved over time (9.1±5.5 in < 2 years, 8.1±5.5 in 2-5 years, 5.7±5.5 in >5 years). Solid stool incontinence and mild constipation improved over time, while moderate to severe constipation remained similar. Moderate to severe frequency episodes were fewer in patients >5 years, but clustering and urgency showed no significant change.

Conclusion/Discussion: Fewer major LARS symptoms were observed in patients with longer postoperative periods, mainly due to improved solid stool incontinence, frequency, and constipation. Clustering, urgency, and gaseous or liquid incontinence remain long-term challenges. Tailored therapeutic approaches may help manage these symptoms.
