Purpose/Background: Electrofulguration remains the preferred surgical method for treating anal and perianal condylomas, demonstrating superior outcomes and lower recurrence rates compared to other treatments.

Objective: This study aims to evaluate the clinical characteristics, treatment modalities, and recurrence rates in patients with anal and perianal condylomas.

Methods/Interventions: Conducted as a retrospective analysis, this study involved 236 patients diagnosed with anal condylomas . Data were extracted from electronic medical records to assess clinical features, treatments administered, and recurrence rates.
Setting: The research was performed in a tertiary-level coloproctology department at a university hospital in Mexico

Results/Outcomes: Patients: The cohort consisted of 180 males (76.3%) with a mean age of 28.9 years and 56 females (23.7%) with a mean age of 35.8 years.Main Outcome Measures: Recurrence rates post-treatment were 27.7% for medical therapy and 14.8% for electrofulguration.

Results: Among the patients, 5.5% had comorbidities, 40.5% were smokers, 41.9% consumed alcohol, and 8.5% used illicit substances. Notably, 19.9% reported having ten or more sexual partners, while 52.1% engaged in anal intercourse; 28.8% were HIV positive. The most common lesion sites were the anal and perianal canals (55.9%). Medical treatment was administered to 22.9% of patients, with imiquimod being the most prevalent (66.6%). Electrofulguration was performed on 39.8% of patients, showing a lower recurrence rate compared to medical treatments.

Conclusion/Discussion: Limitations: The study's retrospective nature and lack of long-term follow-up for some patients limit its findings.

Conclusions: Anal condylomas are more frequent in young men, with imiquimod being the most effective medical treatment. Electrofulguration results in low recurrence rates, and HIV status does not significantly influence recurrence risk. Pre-surgical medical treatment does not reduce postoperative recurrence rates.
