Purpose/Background: Parastomal hernias (PSH) are a frequent complication of ostomies. There is limited data on who is at risk of developing a PSH and factors that necessitate operative repair.

Methods/Interventions: This is a single-institution retrospective cohort study of adult patients who underwent ileostomy, colostomy, or ileal conduit creation between 2018-2020. Patient demographics, operative details, and development of PSH, including time and method of diagnosis, were collected. The primary outcome was frequency and acuity of operative repair of PSH. Univariate analysis was performed to determine risk factors for PSH development and urgent repair.

Results/Outcomes: PSH was diagnosed in 696 (22.9%) of 3029 adults. Mean time to PSH development was 323 (2-1551) days. Among patients who developed PSH, 253 (36%) had ileostomies, 393 (57%) had colostomies, and 49 (7%) had ileal conduits. Of these, 270 (38.9%) patients had their PSH repaired, of which 34 (12.6%) were repaired urgently. Female sex correlated with higher rates of developing PSH (25.7% vs 20.4%, p = 0.001) and requiring urgent operative repair, although this did not reach statistical significance (15.5% vs 9.0%, p = 0.108).

Conclusion/Discussion: Better understanding of the natural history of PSH can inform surveillance strategies in at-risk patient populations, ultimately minimizing the need for urgent surgery and maximizing the likelihood of a more durable repair.
