Purpose/Background: Ventral mesh rectopexy (VMR) is widely accepted treatment for rectal prolapse. Whereas, to avoid the mesh related complications, suture rectopexy (SR) is first line treatment for rectal prolapse in our institution. The aim of this study was to evaluate the therapeutic value of suture rectopexy for rectal prolapse.

Methods/Interventions: This is retrospective study of 79 consecutive patients with rectal prolapse that underwent SR from April 2020 to May 2023 at Coloproctology Center Takano Hospital. One was diagnosed as internal rectal prolapse and a case with recurrence was duplicate entry in this study. All SR procedures were completed laparoscopically. Questionnaires for calculation of Cleveland Clinic Florida Fecal Incontinence Score (CCFIS) for fecal incontinence, Constipation scoring system (CSS) and Obstructed Defecation Syndrome Score (ODSS) for constipation were obtained from the patients followed pre and post-operatively. Maximum resting pressure (MRP) and maximum squeezing pressure (MSP) for anal function were also calculated pre and post-operatively.

Results/Outcomes: There were 6 recurrences (7.6%). The recurrence free survival rate at 1 year was estimated with Kaplan-Meier method as 94.9% and at 2 years as 78.9%. Two of 6 with recurrence were male patients. The recurrence rate of male patients was higher than that of female patients (P=0.013). Two had complications with urinary incontinence, and respiratory failure. The median WS for fecal incontinence was reduced to 10.89±5.06 (0 – 18) at baseline and to 7.04±6.04 (0 – 17) at post-operative state (P=0.0067), While the median CSS at pre and post operative states were 3.29±2.90 (0 – 12) and 2.24±1.71 (0 – 6), respectively (p=0.037). The ODSS were 4.66±3.76 (0 – 17) and 4.44±2.47 (0 – 10), respectively (p=0.74). The MSP was increase to 116.73±77.03 (19 – 316) at baseline and to 139.3±77.97 (37 – 299) at post-operative state (P=0.020),

Conclusion/Discussion: Suture rectopexy is effective for patients with rectal prolapse. However, we must consider whether the indication for male patients is worthwhile.
