Purpose/Background: Obstructed defecation syndrome (ODS) in constipated patients represents a constellation of symptoms that include
excessive straining at stool, a sense of incomplete evacuation, a feeling of perineal fullness, and the need to assist evacuation by self-digitation.Rectocele is defined as a hernia of the rectum with protrusion of the anterior rectal wall through the posterior wall of the vagina, which is one of the most commonly encountered pathologies in ODS patients.A variety of surgical methods have been developed for the treatment of rectocele with different surgical approaches. The short-term effect is acceptable, but the long-term effect is gradually weakened, and there is even the possibility of recurrence. At present, there is no consensus on the best treatment of rectocele. Transvaginal repair of rectocele(TVRR) is a common and effective surgical method for the treatment of low or mid rectocele.This study was to investigate the short-term and long-term efficacy of modified TVRR in patients with moderate and severe rectocele.

Methods/Interventions: This retrospective study was conducted at the Nanjing Hospital of Chinese Medicine Affiliated to Nanjing University of Traditional Chinese Medicine.Twenty-one patients with rectocele meeting the surgical indications underwent modified TVRR from July 2021 to August 2023, and constipation severity score(CSS), postoperative complications and recurrence were assessed at 3, 6,12 and 24 months after modified TVRR.

Results/Outcomes: The average operation duration was (83.14±30.39) min. The average length of hospitalization was (14.10±2.34) days. One patient was lost to follow-up.The overall effective rates of 20 patients at 3, 6 and 12 months after surgery was 100% (20/20)， 90%(18/20) and 80%(16/20), respectively. 15 patients completed 24 months of follow-up, of which 12 （80%）patients still had good results.No postoperative complications occurred.

Conclusion/Discussion: For patients with moderate and severe rectocele, after failure of conservative treatment,and meet the surgical indications. Modified TVRR has good short-term and long-term efficacy in improving constipation symptoms.
