Purpose/Background: To observation the safty and short-term efficacy of moldified Turnbull-Cutait pull-through hand-sewn coloanal anastomosis

Methods/Interventions: Methods：
The study was a retrospective observational study from Jan 2024 to Oct 2024 in Shuguang Hospital Affiliated to Shanghai University of Traditional Chinese Medicine. patients with low rectal cancer who received moldified Turnbull-Cutait pull-through hand-sewn coloanal anastomosis were included.
Interventions：
All patients underwent modified Turnbull-Cutait pull-through coloanal anastomosis.
(1)exteriorization of a segment of left colon through the anal canal.
(2)Severed mesocolic at the level of the anal canal section
(3)Perform a hand-sewn coloanal anastomosis was perform,
(4)after 10 to 14 days, the colon necrosis and exfoliation, If not, resected the exteriorized colon(with no anesthesia).

Results/Outcomes: 12 patients, 8 men and 5 women, with a median age of 67years, were included. All patients did not receive preoperative radiotherapy or chemotherapy, and all patients strongly requested to preserve sphincter. Of the 12 patients, 11 underwent laparoscopic surgery (4-port method) and 1 underwent open surgery. Postoperatively, 6 patients experienced exteriorization colon necrosis , and no colon resection was performed. 6 patients underwent colon resection without anesthesia. Two patients developed stenosis of the anastomosis, with a small finger pass through (colonoscope could not). One patient developed an anastomotic leak, which was repaired with a transverse colonostomy. Postoperative lymph node counts ranged from 12 to 33 (mean 16.8), with the distal resection margin ranging from 0.8 to 2 cm (mean 1.2 cm). Without the use of any drugs, the Wexner score was 3-16 (mean 6.83) at 3 months postoperatively, and the Williams classification was A or B in 3 cases , C in 5 cases，and D in 1 case at 3 months postoperatively.

Conclusion/Discussion: Moldified Turnbull-Cutait pull-through hand-sewn coloanal anastomosis for low rectal cancer is safe, and the nal function is acceptable. The surgery not only avoid a temporary stoma but also Be likely to avoid the exteriorized colon transection .
