Purpose/Background: It has long been argued that patient-reported outcomes, initially developed for research purposes, should be integrated into routine clinical care. There is a lack in clinically-oriented questionnaires that would be feasible to use in daily practice and that would provide useful information to clinicians to help them counsel and treat patients. The development of technologies to collect patient reported outcomes electronically has been transformative. Our objective was to develop a comprehensive, yet concise electronic report for interpretation of PROs in real time in clinic.

Methods/Interventions: In order to integrate PROs into routine clinical practice, we re-evaluated their design and purpose, focusing on three key criteria: 1) ensuring patients’ understanding, 2) promoting patient compliance in completing the forms, and 3) enabling physicians to quickly interpret the results. To enhance patient comprehension, we simplified the language of questions used. To improve compliance, we reduced the questionnaire items and emphasized its role in optimizing clinical care. Finally, we integrated responses in a concise report format that visually highlights key aspects of each patient's responses for efficient review. The main areas of focus are bowel, sexual, urinary function, neuropathy, ability to work, well-being and quality of life. Additionally patient priorities are incorporated into the report, in order to better understand each individual’s needs at every clinic visit. Recovery over time is plotted on a graph that is included in the report that the surgeon reviews before seeing the patient. These core measures are obtained during a patient’s entire treatment course, including baseline assessment, postoperative recovery, and long-term follow up (Figure 1).

Results/Outcomes: The language of the questionnaire items was revised to be simplified, concrete, and straightforward. The number of items, initially over 30, was reduced to fewer than 20, while remaining clinically relevant. Since implementation, the scorecard has been successfully used in clinical practice. While clinician use remains variable, based on individual clinician preference, patient engagement was high, with 70% baseline completion rate. 13,402 patients have received care with use of the electronic reporting PRO tool. Median time to survey completion was 4 minutes.

Conclusion/Discussion: While non-quantitative, our overall experience confirms the predicted benefits on incorporating patient-reported outcomes into clinical practice, such as reducing the time needed to review information, helping focus patient visits, or even influencing how clinicians engage with patients. Administering questionnaires in a busy colorectal clinic, fitting in scoring and reporting within clinic workflow is feasible, however requires a significant ongoing commitment of time and resources by clinicians and staff.
