Purpose/Background: The aim of this study was to evaluate the results of peripheral neuromodulation by tibial nerve stimulation in patients with fecal incontinence.

Methods/Interventions: Between July 2022 and march 2024 118 patients with fecal incontinence refractory to medical treatment were sent to physiotheraphy (PT) with biofeedback. Patients that presented no improvement with conventional PT and biofeedback and presented urinary urgency were submitted to weekly sessions of tibial nerve stimulation.Selection criteria included:fecal incontinence with CCF Scoring system less than 10, low resting and squeeze pressures in anal manometry, no improvement with ballon biofeedback, urinary urgency. Exclusion criteria: associated anismus or other obstructed defecation disorders. Tibial nerve stimulation was performed on a weekly base, with 30 minutes of percutaneous ambulatory stimulation. Primary end point was a reduction in incontinence episodes.Secondary endpoint was improvement in urinary urgency. Parameters evaluated were a change in CCF Scoring system, FIQL instrument and reduction in urinary urgency.

Results/Outcomes: 25 patients ( 28 female) with a mean age of 52 (42-73) years were submitted to a mean of 9 weekly sessions of percutaneous tibial nerve stimulation. All but 2 patients reported an improvement in their weekly bowel accidents as demonstrated by a change in the CCF Scoring System (p < 0.05) in a follow up period of 6 months. No significant differences were observed in any domains of the FI Quality of Life (FIQL)instrument. Urinary urgency improved in 23 patients. (p < 0.05) Patients tolerated well the percutaneous method and there were no complications.

Conclusion/Discussion: In selected incontinent patients, percutaneous tibial nerve stimulation can be a less expensive and less invasive option. However, future and prospective randomized studies with standardized outcome measures and long-term follow-up are still required
