Purpose/Background: Purpose: We analyzed the learning curve of Da Vinci single-port robotic rectal cancer surgery.

Methods/Interventions:

Methods: This was a prospective study of 73 rectal cancer cases who underwent a single port robotic-assisted 9 Anterior Resections (AR), 51 Low Anterior Resections (LAR), and 13 Intersphincteric Resections (ISR) , we used the learning curve analytics by analyzing total operative time ( OT ) , estimated blood loss ( EBL ) and harvested lymph node.

Results/Outcomes:

Results: our study analyzed 73 cases of rectal surgeries done by single port robot, including 9 Anterior Resections (AR), 51 Low Anterior Resections (LAR), and 13 Intersphincteric Resections (ISR), with a gender distribution of 33 males and 40 females, and a mean age of 61 years.
The overall mean operative time was 174.2 minutes, with AR at 145.3 minutes, LAR at 171.2 minutes, and ISR at 213.4 minutes. The mean number of lymph nodes harvested was 19.2, and postoperative pathology report showed a mean PRM of 9.09 and DRM of 3.84.
The learning curve was divided into two phases: Phase 1 (46 cases) with a mean operative time of 187.6 minutes and a peak at 46 cases on the Cusum chart, and Phase 2 (27 cases) with a reduced mean operative time of 155 minutes.
Complications were reported only in Phase 1, including 2 cases of ileus ( treated by TPN ), 5 cases of urinary retention, 1 rectovaginal fistula ( required re-operation ), and 1 anastomosis leak ( treated by pig tail insertion ), with no complications in Phase 2.

Conclusion/Discussion:

Conclusion: The Cusum chart peak at 46 cases signifies the point where the learning curve plateaued, reflecting a improvement with surgeon experience with better outcomes. Overall, the data demonstrates enhanced procedural efficiency and reduced complication rates with increased experience
