Purpose/Background: The integration of virtual reconstruction and surgical planning is proving transformative, particularly in managing complex anal fistulas.

Methods/Interventions: This case study is a 54-year-old obese male patient with a history of five previous surgeries for recurrent anal fistula. The persistent recurrence and complexity of the fistula necessitated an innovative approach to surgical planning. The use of three-dimensional (3D) patient-specific virtual models, underscores the potential benefits of advanced imaging techniques in preoperative planning. In this case, magnetic resonance imaging (MRI) data was utilized to construct a detailed 3D model of the patient's anatomy. This model provided a comprehensive view of the fistula's trajectory and its relationship with surrounding structures, facilitating precise surgical planning. The virtual model allowed for a meticulous assessment of the fistula's secondary tracts and abscess extensions and its impact on adjacent tissues.
This approach not only aids in visualizing complex anatomical relationships but also enhances intraoperative navigation, reducing surgical time and improving outcomes.
