Purpose/Background: Hartmann’s type procedure (sigmoid colectomy with end-colostomy) is most commonly a damage control operation performed for severe cases of diverticulitis. There is no consensus on the optimal timing for Hartmann’s reversal. Identifying factors that contribute to complications and prolonged Hartmann’s reversal length-of-stay (RLOS) can help guide pre-operative decision making. We evaluated factors that may impact RLOS and the likelihood of complications with patients that underwent Hartmann’s reversal within 180 days from the initial procedure.

Methods/Interventions: A retrospective cohort study was conducted using data from the HCA Healthcare Database on patients who underwent Hartmann’s reversal. Logistic regression was used to analyze the likelihood of 180-day complications and negative binomial regression was applied to assess the predictors of (RLOS).

Results/Outcomes: There were 9807 patients identified who underwent a Hartmann’s procedure. 1189 were excluded because of: missing demographic data (112), duplicate encounters (116), and Hartmann’s reversal without record of index procedure (952). A total of 8618 unique patients were included in the data set. 693 (8%) of these patients had a Hartmann’s reversal within 180 days of the index procedure. The cohort median age was 62 years (range 25-90), predominantly female (55.1%) and white (83.7%). Four (0.6%) patients expired and 266 (38.4%) experienced a complication. None of the variables analyzed, including age (p=0.83), sex (p=0.74), race (p=0.42 for White vs Other), or time to reversal (p=0.64) predicted complications. However, increasing age was predictive of increased RLOS (IRR 1.01 per year, p=0.02). Other factors including sex, race, and time to reversal were not predictive of RLOS.

Conclusion/Discussion: For patients who underwent Hartmann’s reversal within 180 days of the index operation, age was a predictor of longer RLOS.
