Purpose/Background: In recent studies, the rates of mesh complications with synthetic and biological meshes were reported as 1.87% and 0.22%, respectively. While various mesh-related complications have been reported in previous research, we aimed to assess the occurrence of complications in Laparoscopic Ventral Mesh Rectopexy (LVMR) procedures performed at our institution.

Methods/Interventions: From September 2013 to November 2023, patients who underwent LVMR were included. Patient demographics and surgery-related data were collected. Surgical complications, including mesh-related issues, were documented. In order to assess functional outcomes, severity grade of constipation, fecal incontinence, and pelvic pain were collected before and after surgery

Results/Outcomes: Total 264 patients with IRP (n=103, 39.0%) or ERP (n=161, 61.0%) underwent LVMR. The mean age of the patients was 65.06±13.63 years. The mean operation time was 124.75±32.52 minutes, and the mean hospital stay was 5.48±1.74 days. The mean follow-up was 30.20 ±28.54 months. There were no mesh-related complications. Eight patients (3.08%) experienced minor complications including incisional hernia and voiding difficulty. Following LVMR, constipation symptoms improved (49.7%) or were cured (33.1%) in patients with obstructive defecation syndrome before surgery. Fecal incontinence also improved (52.1%) or was cured (40.0%). Chronic pelvic pain improved or was cured in 86.5% of patients. Overall satisfaction was reported at 91.3% among patients who responded to the satisfaction survey.

Conclusion/Discussion: Our study demonstrates that LVMR can be performed safely without mesh-related complications. LVMR was performed in patients excluding the high-risk group for mesh-related complications. In cases where mesh insertion was deemed inappropriate, conversion to suture rectopexy or resection rectopexy was decided during the procedure. Adhering to these principles ensures the safety of LVMR as a surgical approach.
