Purpose/Background: The animal model of anal fistula is essential for exploring the pathogenesis and developing innovative treatments for anal fistulas. However, current animal models are not well-established, with most focusing on Crohn's disease-related anal fistulas and lacking mainstream modeling methods that align with clinical symptoms. In China, epidemiological studies have shown that a high-fat, high-sugar diet, alcohol consumption, and prolonged exposure to a humid environment are high-risk factors for the development of anal fistulas, referred to as the "Damp-heat pouring downward" type. This study aims to establish a rat model of anal fistula that aligns with the common clinical etiology and symptoms by improving upon the modeling methods used by other researchers based on the most common clinical factors.

Methods/Interventions: Sixteen SPF grade male SD rats were randomly divided into normal and model groups, with 8 rats per group. The model group was fed with wet bedding, high-fat, high-sugar diet, and subjected to alternate-day intragastric administration of white wine to preliminarily establish a susceptible rat model for anal fistula. Subsequently, stainless steel needles were inserted into one side of the rat's perianal internal and external sphincters to continue the model construction. After 30 days, perianal conditions of the rats were observed, and ELISA was used to detect serum inflammatory factors. Perianal ultrasound and HE staining were performed to observe fistula formation.

Results/Outcomes: The success rate of anal fistula modeling in this study was 100%. Following modeling, rats exhibited increased perianal secretions, and serum levels of tumor necrosis factor-α (TNF-α) and interleukin-8 (IL-8) were significantly elevated (P < 0.05). Perianal ultrasound revealed distinct linear hypoechoic structures with clear boundaries. HE staining showed the formation of fistulas characterized by significant fibrosis, with infiltration of inflammatory cells and granulation tissue observed in the surrounding area.

Conclusion/Discussion: The use of a humid environment, high-fat high-sugar diet, white liquor gavage, and stainless steel pin loosely hung can successfully establish a rat model of anal fistula that closely resembles clinical symptoms, with a high success rate. This model provides a reasonable and reliable animal model for basic research on anal fistulas.
