Purpose/Background: Myofascial Pelvic Pain Syndrome is a disease characterized by pelvic pain which involves every muscle group of the pelvic floor, the myofascial pain syndrome can involve any muscle in the body and the main diagnostic criteria involve trigger points and tenderness bands which provoque pain with pressure or movement.
Up to 50% of women older than 35 years can suffer this syndrome through their lives, it is one of the most common causes of chronic musculoskeletal pain and also among most overlooked ones.

Methods/Interventions: Retrospective, transversal, observational and descriptive study with 51 patients of the Anorrectal Physiology Unit of Hospital General de México which is a Tertiary Care Centre. This patients were received by the Unit in the lapse of 7 months sent to us by Coloproctology Service and sometimes Gastroenterology or General Surgery.
Every Patient was studied via Anorrectal High Resolution Manometry and Endo Anal Ultrasound. Every patient answered a thorough medical history including the beginning of symptoms and every patient was explored thoroughly physically with special attention to pelvic floor through anal tact, vaginal tact, bimanual tact and pressure of posible trigger points among the pelvic floor muscles transvaginaly and transanally.

Results/Outcomes: The time between de beginning of the pain and until the diagnosis was on average of 19 months, this indicates long pain periods and probably multiple treatments before reaching the diagnosis of Myofascial Pelvic Pain Syndrome. Every Patient was studied via Anorrectal High Resolution Manometry and Endo Anal Ultrasound, the most prevalent manometric alterations were in anorrectal sensibility.
Between the patients studied we found 28 with Myofascial Pelvic Pain Syndrome criteria, they had also the longest periods of time to treatment since the beginning of the pain and was initially sent for other causes of pelvic pain, only 16% of them were sent with a probable diagnosis of MPPS.

Conclusion/Discussion: Myofascial Pelvic Pain Syndrome is an underdiagnosed entity and is often mistaken by other proctological diseases, it is our responsibility to consider it among common causes of chronic pelvic pain specially in patients who had not reached a definite diagnosis; it can be achieved through a thorough physical examination and history.
