Purpose/Background: Rationale/

Objectives: This paper aims to discuss a case on colobronchial fistula, to discuss the importance of systemic evaluation for diagnosis, and its multidisciplinary management and strategy.

Methods/Interventions: This study follows the case of a 62-year old male, the course of his hospital stay as well as the preoperative, intraoperative and post-operative condition of the patient. Personal interviews and daily bedside rounds were done to ensure continuity of care and refrain from biases.

Results/Outcomes: Histopathology of the specimen taken intraoperatively show diverticulosis and submucosal edema of the cecum, ascending and transverse colon as well as congestion of the terminal ileum. The specimen also noted fibrocollagenous and fibroadipose tissues with mild chronic inflammation. This adds to the studies supporting that diverticulosis is a possible causes of colobronchial fistulas.

Conclusion/Discussion: Colobronchial fistulas are rarely encountered and can present diagnostic challenges. This case underscores the importance of considering a fistula in patients with both respiratory and gastrointestinal symptoms post-surgery. Management often involves a multidisciplinary approach, including surgical and supportive care. Imaging such as barium enema and/or CT scan with multiplanar reconstruction are valuable for the confirmation of CBF. Surgery, however, is still the foundation of treatment. Currently, there are no published reports and research readily available on this case especially in the Philippines. This report may be informative and should initiate discussion and research in creating guidelines in the management of these patients.
