Purpose/Background: Venous thromboembolism (VTE) events are a common and significant health problem in the United States that can lead to serious post-operative morbidity and even death. However, there is a lack of data regarding VTE risk in colorectal surgery for pelvic floor and motility disorders (PFM). This study aimed to determine the incidence of VTE through 90-days following surgery for PFM and identify associated risk factors.

Methods/Interventions: Adult patients undergoing rectal prolapse repair, subtotal colectomy with ileorectal or end ileostomy formation and sacral nerve modulation undertaken for fecal incontinence between January 1, 2006 and December 31, 2023 at a single institution were identified. VTEs within 90-days of surgery were identified using diagnosis codes in the electronic medical record and consisted of extremity deep venous thromboses, portomesenteric venous thromboses, and pulmonary emboli.

Results/Outcomes: A total of 818 patients were identified in this study: 73.1% underwent rectal prolapse repair, 16.9% underwent subtotal colectomy and 10% underwent sacral nerve modulation. The incidence of any VTE was 1.6% at 90 days, with the majority diagnosed greater than 30 days from surgery (61.5%). Portomesenteric venous thromboses were the most common type of VTE, accounting for 38.5% of events. VTE risk was highest following subtotal colectomy for slow transit constipation at 7.2%, likely secondary to the nature of the operation. Risk of VTE following prolapse procedures was 0.5%. No VTE events were identified following sacral nerve modulation.

Conclusion/Discussion: Overall, the VTE event rate following colorectal surgery for pelvic floor and motility disorders is low. Special consideration might be given to patients undergoing subtotal colectomy for slow transit constipation
