Purpose/Background: Background: Whether preoperative serum carbohydrate antigen 19-9 (CA19-9) adds prognostic value on carcinoembryonic antigen (CEA) on survival in patients with colorectal cancer (CRC) is unknown.

Objective: To explore the prognostic value of preoperative serum CA19-9 in comparison to CEA with colorectal cancer after radical resection.

Methods/Interventions:

Methods: We retrospectively divided the 1604 patients from a prospectively maintained database into four groups based on preoperative serum biomarkers: normal CEA and CA19-9, normal CEA, elevated CA19-9; elevated CEA, normal CA19-9; elevated CEA and CA19-9. We used propensity score matching to adjust for potential baseline confounders between groups. Survival was analyzed using the Kaplan-Meier method, and multivariate analyses were conducted for CA19-9, CEA and other clinicopathological factors.

Results/Outcomes:

Results: After matching, five-year overall survival of the four groups was 100%, 73.3%, 71.1%, and 50.4%, respectively (log-rank p < 0.001). CA19-9, CEA, and TNM stage were independent risk factors for overall survival. No interaction between CEA and CA19-9 on survival were detected (p for interaction=0.091). CA19-9 was associated with CEA and TNM stage. (both p < 0.001).

Conclusion/Discussion:

Conclusion: Preoperative CA19-9 is an independent prognostic factor in patients with CRC. Combination of preoperative CA199 with CEA provides additional prognostic value after radical surgery.
