Purpose/Background: Diverting loop ileostomy closures are often the last surgery patient’s require after a long course of complex care for cancer, diverticulitis, inflammatory bowel disease, or other disease process. They are often shorter, more simple procedures. However, postoperative ileus, anastomotic leak, or other postoperative complications can be devastating. Further, laparoscopic vs. Open reversal does not fit the standard paradigm we think of in minimally invasive surgery. The ostomy must be resected. This often is the only incision needed to complete the operation. We aim to assess the utility the laparoscopic surgery in diverting ostomy reversal. Our secondary aim is to investigate for areas for improvement at our center for these patients.

Methods/Interventions: Institutional review board approval was obtained prior to data collection. All patients who underwent diverting loop ileostomy closure from January 2020 to September 2024 were identified. We retrospectively collected patient demographics, operative information, treatment history, diagnostic history, and complications.

Results/Outcomes: A total of 118 patients were identified. 58 had an open procedure while 60 underwent laparoscopic closure. There was no difference in age, gender, median number of comorbidities, delay in time to surgery, blood loss, postoperative ileus, or time to flatus. For the primary outcome, when comparing the open to laparoscopic procedure there was no difference in the median length of stay (3 days [2 - 4 days] vs. 2.2 days [2 - 3.2 days]; p=0.2784). There was a shorter amount of operative time in the open group (77 minutes [65 to 92 minutes] vs. 108 minutes [74.5 minutes to 139 minutes]; p=0.0013). A total of 11 patients in the cohort had an ileus. When comparing those who had an ileus to those who did not have an ileus, the median time to surgery was significantly longer (6.3 months [4 to 17 months] vs. 3.7 months [3 to 6 months]; p=0.0235)

Conclusion/Discussion: The only significant difference between open and laparoscopic closure was the length of surgery. Open surgery alone without diagnostic laparoscopy remains a safe alternative to routine laparoscopy in diverting loop ileostomy reversal. Delay to ostomy closure was the only significant difference between those who developed POI vs. those who did not. Future research is needed to further risk factors and strategies to mitigate postoperative ileus and other complications.
