Purpose/Background: Colon cancer is a significant contributor to cancer-related mortality in the U.S., with an increasing incidence in early-onset cases. Hispanic populations are disproportionately affected, often diagnosed at younger ages compared to other racial groups. However, limited research stratifies Hispanic patients by country of origin. This study aims to examine differences in clinical presentation and survival outcomes among Hispanic subpopulations, compared to non-Hispanic Whites, to identify disparities and guide improvements in patient care.

Methods/Interventions: This retrospective cohort study utilized NCDB data (2008–2021) to analyze Hispanic patients diagnosed with colon cancer, stratified by country of origin (Mexico, Puerto Rico, Cuba, and other Latin American countries) and compared to non-Hispanic White patients. Chi-square tests (95% CI) assessed demographic and tumor characteristics. Cox proportional hazards regression evaluated survival outcomes, adjusting for cancer stage, Charlson-Deyo score, tumor size, age of onset, sex, insurance status, and origin. Statistical significance was set at p < 0.05.

Results/Outcomes: A total of 822,628 patients with colon cancer were analyzed, including 19,120 Hispanic patients across various subpopulations. Mexican patients represented the largest group (39.6%). Early-onset colon cancer ( < 45 years) was more prevalent among Mexicans (15.2%) and South/Central Americans (12.0%) compared to non-Hispanic Whites (5.4%). Cox regression showed that advanced stage, higher Charlson-Deyo scores, and being uninsured were associated with worse survival. However, Mexican (HR: 0.748) and South/Central American (HR: 0.728) patients had better survival than non-Hispanic Whites (p < 0.001).

Conclusion/Discussion: This study reveals significant differences in clinical presentation and survival outcomes of colon cancer among Hispanic patients based on their country of origin. Notably, Mexican and South/Central American patients demonstrated better survival outcomes than non-Hispanic Whites, despite being more likely to present with early-onset disease. These findings underscore the importance of understanding ethnic and regional disparities to guide equitable healthcare policies and improve outcomes for all Hispanic patients with colon cancer.
