Purpose/Background: The K pouch (continent ileostomy) is a technically challenging and complex procedure with expertise limited to few surgeons and widely known to be associated with significant complications. The ileal pouch-anal anastomosis (IPAA) is sometimes an alternative and more commonly performed procedure following proctocolectomy. How the complications between the two procedures compare have not been elucidated.

Methods/Interventions: All patients who underwent either a primary (not revision or conversion) J pouch or K pouch between 2014-2023 were included from a prospectively maintained database at an academic institution. The following data were collected: patient demographics; postoperative outcomes including length of stay (LOS), readmission, reoperation, mortality, and postoperative complications. Chi-square test was used to compare dichotomous variables, while the t-test or Mann-Whitney test were used for continuous variables, as appropriate. A p-value of < 0.05 was considered significant.

Results/Outcomes: A total of 96 patients with J-pouch and 32 patients with K-pouch creation were included in the study. The median age (46 years (IQR 26-60) vs. 45 years (IQR 32–53), p=0.84) and median body mass index (BMI) (23.7 (IQR 20.6-27.8) vs. 21.6 (IQR 20.4-25.1) kg/m2, p=0.17) were similar for the K- and J-pouch groups. Operating time was greater for K-pouch (4 hours and 53 minutes vs. 3 hours and 53 minutes, p=0.003). Median length of stay was shorter for the J pouch patients (6 (IQR 4-9) vs. 8 days (IQR 7–11), p=0.005) while there was no significant difference in readmission (24% vs 25%, p=0.91).
For 30-day outcomes, anastomotic leak (1% vs 3.1%) and abscess (7.3% vs 6.3%) were similar for J- and K-pouch. There was also no significant difference in ileus (p=0.43), sepsis (p=1.00), obstruction (p=0.26), or venous thromboembolism (p=0.6) between the groups. Reoperation rate was 6.3% in the J-pouch group and 0% in the K-pouch group (p=0.34).

Conclusion/Discussion: Our analysis shows that although K-pouch creation is associated with longer operative times and hospital stay, it can be done with similar 30-day outcomes to J-pouch creation, at an experienced Center.
