Purpose/Background: Measuring the C-reactive protein (CRP) level on post-operative day (POD) 3 has emerged as a sensitive biomarker for anastomotic leak (AL) in patients undergoing colorectal surgeries. However, the utility of measuring CRP has not yet been studied specifically for ileostomy reversal procedures. Our objective is to study whether CRP levels had any predictive value on the post-operative outcomes in patients who underwent ileostomy reversals.

Methods/Interventions: All patients were taken from a prospectively maintained database. We retrospectively reviewed all patients who underwent ileostomy reversals at an academic tertiary care center from July 2015 to November 2023. Demographic information and outcome measures, including length of stay (LOS), anastomotic leakage (AL), time to first bowel movement, surgical site infection (SSI), readmission, reoperation, and death, were analyzed.

Results/Outcomes: A total of 79 patients met the criteria for this study, including 30 patients who had their CRP measured post-operatively following their ileostomy reversal and 49 patients who did not have their CRP measured. There was no significant difference in the demographics of the CRP group vs the Non-CRP group in sex (p = 0.383), age (p = 0.164), body mass index (BMI) in kg/m² (p = 0.587), or Charlson Comorbidity Index (CCI) (p = 0.637). We also found no significant differences in our patients’ type of ileostomy (p = 0.207), indication (p = 0.430), or approach (p = 0.460). The results of our analysis showed that there were no significant differences in the outcome measures between the CRP and Non-CRP groups, as seen in Table 1. Additionally, a subgroup analysis of the CRP group was conducted comparing the High-CRP and Low-CRP groups based on our institutional cutoff of 170 mg/L. This analysis showed no significant differences in the same outcomes measured.

Conclusion/Discussion: Our results demonstrated that there was no clinical benefit in measuring CRP post-operatively in ileostomy reversal patients. While it has been shown to be a helpful metric in other colorectal procedures, CRP does not appear to have any significant predictive value for post-operative outcomes in ileostomy reversals.
