Purpose/Background: To demonstrate a novel technique for total mesocolic excision for Rectal cancers in performing a low anterior resection laparoscopically

Methods/Interventions: The Patient was placed in a modified Loyd Davies position and pelvic ports were placed. the dissection began with ligation of the inferior mesenteric artery followed by medial to lateral dissection of the fascial planes. Toldts fascia and Gerotas fascia identified. the inferior mesenteric vein ligated next followed by splenic flexure mobilization. the lateral dissection was done next followed by pelvic dissection. sharp posterior dissection done in the Holy plane of Heal by identifying the inferior hypogastric nerves. Linear staplers used for resection and circular stapler used for anastomosis.

Results/Outcomes: the operating time was 150 minutes with 200 ml of blood loss. Patient was started orally on Post Operative day 1 and discharged on PostOperative day 5. The pelvic drain was removed on Post operative day 5.

Conclusion/Discussion: A complete anatomical approach to performing a low anterior resection for rectal cancer with a nerve sparing technique.
