Purpose/Background: Combined endoscopic laparoscopic surgery (CELS) was developed for unresectable polyps to avoid segmental colectomy and is a well-established technique in the literature. Here we present a patient with an endoscopically unresectable appendiceal orifice polyp that was managed with combined endoscopic and robotic surgery (CERS).

Methods/Interventions: A combined endoscopic and robotic appendectomy was performed on a 67-year-old patient who had previously undergone endoscopic resections of an appendiceal orifice polyp, which had recurred. The case was performed primarily by a resident, with an attending assisting on the dual console.

Results/Outcomes: Robotic and endoscopic visualization allowed an appropriate cecal margin for resection of the polyp, without compromise of the ileocecal valve. This approach was well tolerated by the patient, who was discharged on the same day, and proved to be a valuable opportunity for resident robotic education.

Conclusion/Discussion: Combined endoscopic robotic surgery is a modification of the more common combined endoscopic laparoscopic surgery. This is an important tool in the armamentarium of the colorectal surgeon to target large or unresectable appendiceal and cecal polyps. Given the increasing prevalence of robotic surgery it is important that surgeons are comfortable adapting known techniques for new technology.
