Purpose/Background: Ileostomy is the most common temporary diversion. Even if temporary, a poor siting or a poor construction of an ileostomy can contribute to morbidity, suffering and lead to increased primary and secondary healthcare cost. The solution to this problem is to create a well-shaped loop ileostomy with a symmetric nipple that looks and behaves like and end ileostomy.

Methods/Interventions: Video demonstration of the technique of creating a loop ileostomy in functional end ileostomy configuration.

Results/Outcomes: Construction principles include:
• Trephine/skin opening of similar diameter as bowel (larger opening may result in lateral tenting of nipple)
• Cylindrical appearance of ~ 2.5cm nipple
• Minimizing efferent limb
• Nipple formation to focus on afferent limb

Conclusion/Discussion: The result of this technique shows a symmetric, roughly 1 inch nipple, that can be easily managed with a stoma appliance.
